We also take it upon ourselves to coordinate your home care from other providers such as home pharmacies and medical equipment companies. If you have orders from multiple doctors, we collect all that information and incorporate it into one home health plan of care. By utilizing multiple specialties on our team and coordinating the work of your other healthcare providers, we provide a cohesive, whole-person approach designed to give you the best possible medical outcomes.
